Our Services
We provide Supported Community Living (SCL) services, which are within the Iowa Medicaid's Home and Community Based Services (HCBS) umbrella. Currently we serve individuals with Intellectual Disability (ID) and/or Brain Injury (BI). We utilize the host home model of service provision.
Host Home Model
A host home refers to a community-based rented or owned family home setting that is the primary residence of the host home provider. A host home provider is a person contracted by an agency such as Tandem Services to provide Supported Community Living (SCL) services to clients.
In the host home model of service provision, an individual with an intellectual disability and/or brain injury resides in the host home and receives daily services from the contractor. Tandem Services provides assistance and oversight to the contractor.
Host Homes
In our Host Homes, adults with intellectual and developmental disabilities, mental health challenges, brain injury, and other needs live in private family homes with dedicated caregivers.
Adults become part of the family, and with their caregiver's support they build new skills, find new hobbies, and enjoy community activities. This personalized approach creates a nurturing environment where individuals can thrive.
